Publish latest version of sqlcommenter-knex to NPM

It looks like some of the latest changes in `sqlcommenter-knex` (like https://github.com/google/sqlcommenter/pull/146) weren't published to NPM.

If I install the package from NPM and look at the code, it still has references to `opencensus`, while the code on `master` no longer has those references.

Could you please publish the latest changes to NPM?
