Add --header-text and --instruction-file-path for custom content

Add support for custom content injection: --header-text <text> (custom text at beginning of output) and --instruction-file-path <path> (path to file containing custom instructions). These features allow users to add context/instructions to output, which is important for AI prompt engineering.
